Life Coaching Consent & Liability Waiver

This agreement outlines the terms and expectations of the coaching relationship between the Client and Coach. Coaching is a collaborative process designed to support personal growth, well-being, and positive change.

Coaching is not therapy, counseling, medical treatment, or a substitute for professional healthcare or mental health services. It is a future-focused approach aimed at helping individuals gain clarity, take action, and create meaningful transformation in their lives.

1. Nature of Life Coaching

  • Coaching is a thought-provoking and creative process designed to support you in achieving your personal goals.

  • The Coach is not a licensed medical or mental health professional and does not diagnose, treat, or cure any medical, psychological, or emotional conditions.

  • Coaching is future-focused, emphasizing growth, self-awareness, and positive action. It does not replace medical, psychological, legal, or financial advice.

  • The Client understands that coaching results depend on their commitment and willingness to take action.

2. Client Responsibilities

  • The Client is responsible for their own decisions, actions, and well-being. Coaching is a self-directed process, and results depend on the Client’s participation.

  • The Client agrees to communicate openly and honestly throughout the coaching relationship.

  • The Client acknowledges that coaching does not guarantee specific outcomes but provides tools, support, and guidance for transformation.

3. Confidentiality

  • All coaching conversations and records are confidential and will not be shared without the Client’s written permission, except in the following circumstances:

    • If required by law (e.g., threats of harm to self or others, court orders, suspected abuse).

    • If the Coach is legally obligated to disclose information for safety or ethical reasons.

  • The Client agrees not to record coaching sessions without mutual consent.

4. Liability Waiver

  • The Client acknowledges that they are solely responsible for their decisions and actions as a result of coaching sessions.

  • The Coach is not liable for any direct or indirect consequences of the Client’s choices.

  • The Client understands that coaching is not a replacement for medical, psychological, financial, or legal advice and agrees to seek professional support in those areas when needed.

5. Fees & Cancellation Policy

  • Coaching fees are agreed upon before sessions begin. Payments are non-refundable, except as stated in the coaching agreement.

  • Cancellations require at least 24 hours' notice to reschedule. Missed or last-minute cancellations may result in a forfeited session.

6. Termination of Coaching Relationship

  • Coaching is a voluntary process. Either the Client or Coach may end the coaching relationship at any time with written notice.

  • If the Coach determines that coaching is not the best fit for the Client’s needs, appropriate referrals may be provided.

7. Acknowledgment & Agreement

By signing below, the Client acknowledges that they have read, understood, and agree to the terms of this agreement. The Client releases the Coach from any claims, liability, or damages related to coaching services.
